In-home care means a trained caregiver comes to your parent's home to help with daily activities — meals, bathing, dressing, companionship, and transportation — so they can stay home safely. California's IHSS program (In-Home Supportive Services) can fund in-home care for Medi-Cal-eligible seniors at no or very low cost to the family.

In-home care works best when your parent is safe at home with some support, wants to maintain their independence and routine, and has family nearby who can check in. When care needs increase — especially overnight supervision, 24/7 monitoring, or complex medical needs — a residential setting like assisted living or board & care often becomes safer and sometimes even more cost-effective when you factor in the hours of care required.

In-home caregivers in California are typically provided through licensed home care agencies (HCA) regulated by the California Department of Social Services (CDSS). Independent contractors are also common but carry more risk for families. IHSS (In-Home Supportive Services) is California's Medi-Cal-funded program — a county social worker assesses your parent's needs and authorizes a set number of care hours per month. IHSS is separate from agency-provided care and requires Medi-Cal enrollment and a county needs assessment.
